After being denied access to hormone treatment, an American prisoner who wanted gender-affirming care castrated herself in a Utah prison.
The Department of Justice (DOJ) is suing the state of Utah and its corrections department for violating the Disabilities Act.
The unidentified prisoner, who was diagnosed with gender dysphoria in June 2022 by a doctor hired by the Utah Department of Corrections, had been requesting access to hormones, women’s housing, women’s underwear and makeup for the past two years.
The inmate’s mental health began to deteriorate after she was denied accommodation and castrated herself during her state of deep mental anguish.

The federal investigation determined that the state correctional agency put up “unnecessary barriers” that prevented the inmate from receiving treatment for her severe gender dysphoria.

Although the doctor approved the inmate’s treatment for her condition in 2022, treatment was not provided until January 2023, seven months after her diagnosis was made.
The Justice Department investigation claims the prisoner found it more difficult to receive treatment for her condition due to the nature of her diagnosis.
The investigation also determined that prison officials intentionally delayed the prisoner’s access to hormone treatment, even as her mental health worsened.
Gender dysphoria, defined by the Department of Justice as “a serious medical condition” that causes “significant distress or impairment due to an incongruence between your gender identity and your assigned sex,” is recognized as a disability under the Act of Disability.
Therefore, inmates living in a correctional facility have the right to receive treatment for this condition.
Although the doctor approved the prisoner’s treatment for his condition in 2022, he was not provided treatment until January 2023.
The investigation also determined that prison officials intentionally delayed the inmate’s access to hormone treatment, despite his worsening mental health.
According to the Department of Justice, the corrections department “failed to make reasonable modifications to its policies and practices to address the complainant’s gender dysphoria.”
They also call for the implementation of anti-discrimination training and policy changes for all Utah correctional officers.
The inmate will be duly compensated by the correctional department for the damages.
