Queensland Police have issued an urgent Amber Alert for a four-week-old baby who may be at significant risk.

The boy, from Fortitude Valley in Brisbane, was last seen on Sunday.

A 37-year-old woman known to the baby was last spoken to at midday Thursday.

He is believed to be traveling on foot around the Brisbane city area.

She is described as a Pacific Islander, approximately 168cm tall, with a proportional build and brown hair and brown eyes.

Anyone with information about the missing baby can call 131 564 or 000.
