In 2008, a British police officer was accidentally shot by a colleague during training.

PC Ian Terry played the role of a criminal who fled in a car when he was fatally shot in the chest with a CS gas grenade designed to overpower suspects in Manchester.

In 2016, a Florida cop dressed as a ‘villain’ shot and killed a 73-year-old mother while role-playing – because he was loading live rounds instead of blanks.

Agent Lee Coel, 28, ‘accidentally’ murdered retired librarian Mary Knowlton in front of 34 people, including her husband, during the demonstration in Punta Gorda, Florida.

The purpose of the lesson was to teach people when not to shoot.
