A frantic search is underway to find a frail man missing in western Sydney.
Denis Burke, aged 78, was last seen at Concord Hospital on Sunday, July 18 and has not been seen since.
Officers from Burwood Police Area Command have been contacted and are investigating.

Police are concerned for Mr Burke’s well-being as he has a medical condition.
Mr Burke is known to travel on the rail network and regularly visit the Sydney CBD.
He is described as a Caucasian in appearance with light skin and gray hair, and is about 175 cm tall and of thin build.
Mr Burke was wearing a black puffer jacket and black jeans when he was last seen.
